Job Description

Implement and manage transitional care (Aftercare) pilot programmes to support clients with mental health conditions and complex needs from hospital to community setting and vice versa

  • Responsible for the development of assigned programmes which includes coordinating projects from end-to-end and managing the administration of pre-award to post-award activities
  • Engage and work with relevant stakeholders to ensure seamless collaboration between hospital and community partners in supporting clients requiring transitional care
  • Review model of care based on the local and overseas trends. This includes understanding ground practises through programmes’ learning  and recommendation
  • Assist in the development of capability and competencies by the Restructured Hospitals providers to the community partners with the aim to better support clients requiring transitional care


Job Requirements:

  • Basic degree in any discipline
  • Minimum 2 year of working experience in community services or partners engagement
  • Experience in implementing new programmes and      healthcare industry
  • Proficient in MS Office applications
  • Good project management, organisational and strong inter-personal skills with both internal and external customers
  • Able to communicate effectively verbally and in writing
  • Team player with high level of initiate
  • Proactive, self-motivated, independent
  • Able to multi-task, work in fast-paced and dynamic work environment
