Job Description
Implement and manage transitional care (Aftercare) pilot programmes to support clients with mental health conditions and complex needs from hospital to community setting and vice versa
- Responsible for the development of assigned programmes which includes coordinating projects from end-to-end and managing the administration of pre-award to post-award activities
- Engage and work with relevant stakeholders to ensure seamless collaboration between hospital and community partners in supporting clients requiring transitional care
- Review model of care based on the local and overseas trends. This includes understanding ground practises through programmes’ learning and recommendation
- Assist in the development of capability and competencies by the Restructured Hospitals providers to the community partners with the aim to better support clients requiring transitional care
Job Requirements:
- Basic degree in any discipline
- Minimum 2 year of working experience in community services or partners engagement
- Experience in implementing new programmes and healthcare industry
- Proficient in MS Office applications
- Good project management, organisational and strong inter-personal skills with both internal and external customers
- Able to communicate effectively verbally and in writing
- Team player with high level of initiate
- Proactive, self-motivated, independent
- Able to multi-task, work in fast-paced and dynamic work environment
