Job Description

Implement and manage transitional care (Aftercare) pilot programmes to support clients with mental health conditions and complex needs from hospital to community setting and vice versa

  1. Responsible for the development of assigned programmes which includes coordinating projects from end-to-end and managing the administration of pre-award to post-award activities
  2. Engage and work with relevant stakeholders to ensure seamless collaboration between hospital and community partners in supporting clients requiring transitional care
  3. Review model of care based on the local and overseas trends. This includes understanding ground practises through programmes’ learning  and recommendation
  4. Assist in the development of capability and competencies by the Restructured Hospitals providers to the community partners with the aim to better support clients requiring transitional care

Job Requirements:

  • Basic degree in any discipline
  • Minimum 2 year of working experience in community services or partners engagement
  • Experience in implementing new programmes and healthcare industry 
  • Proficient in MS Office applications
  • Good project management, organisational and strong inter-personal skills with both internal and external customers
  • Able to communicate effectively verbally and in writing
  • Team player with high level of initiate
  • Proactive, self-motivated, independent
  • Able to multi-task, work in fast-paced and dynamic work environment
