Job Description

  • Assess client's medical, physical, social and mental health status, to better coordinate and provide care that is timely, safe and client-centered
  • Support the team with clinical/case management skills to ensure that vulnerable, frail and complex clients are appropriately managed
  • Facilitate multiple aspects of care including clinical case discussion, case coordination and navigation of the care system
  • Assist clients and caregivers to make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop, implement, review and coordinate care plans in collaboration with community partners to support clients and caregivers' needs to achieve wellness and autonomy
  • Conduct home visits for comprehensive care assessment, incorporating restorative, physical, medical, psychological, cultural and social dimensions of care needs
  • Work in collaboration with healthcare partners and front-line agencies for accurate assessment and recommend appropriate follow-up actions in facilitating smooth care transition
  • Develop and maintain effective working relationships with the medical care team and multiple agencies throughout the case management process
  • Handle case assignments, review case progress and determine case closure
  • Perform case documentation and data collation to ensure timely submission of reports and information

Job requirements:

  • Degree in Nursing, Allied Health, Psychology, Social Work or Counselling
  • Minimum 3 years of experience preferably in health, mental health and/or social sector in Singapore
  • Good knowledge of geriatrics and community services, or experience with case management and psychological aspects of care preferred
  • Commitment and compassionate in serving seniors
  • Demonstrate problem solving skills and ability to multi-task
  • Effective communication and interpersonal skills, with good teamwork
  • Excellent organisational and time management skills
  • Ability to work in a fast-paced and dynamic work environment
  • Proficient in Microsoft Office applications and basic statistics
