Job Description

Primary Objectives of Position:

  1. Lead the CREST team to ensure service delivery is in accordance to the intended program objecives and care model for CREST program
  2. Provide supervision and guidance to the CREST team to achieve outreach goals to increase mental health and dementia awareness, promote early identification and link those at risk and with needs to the appropriate support and resources, and provide follow up support
  3. Responsible for monitoring the team's progress and performance

Job Responsibilities:

  1. Plan, implement & monitor the outreach, care process, referral and follow-up workflow
  2. Provide supervision to team members to provide care and support, in alignment with the overall CREST program objectives, adhering to the relevant ethical and legal guidelines
  3. Provide care coordination and service linkages to clients with or who are at-risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support, through follow-up calls and home visits as required
  4. Establish and develop network with other stakeholders like community partners and/or related agencies to build collaborative partnerships, better facilitate care and support for clients and caregivers
  5. Initiate, design, develop and implement initiatives in the development of dementia-friendly communities by mobilizing community resources, other social service providers and volunteers
  6. Lead team in programme evaluation & development, conduct local needs analyses for community work practice on trends and issues relating to dementia/mental health services and needs of clients and their families
  7. Lead the development of a caregiver support network and facilitate group work sessions
  8. Ensure programme data and documents are kept and maintained, in accordance to the CREST programme requirement, as well as the prevailing personal data protection and security guidelines
  9. Document, track & manage team reporting and collation of client statistics and data for accurate and timely submission to stakeholders
  10. Contribute to organization/department’s projects and committees, as and when required


- Bachelor/ Postgraduate Diploma/ Master in Social Work

- At least 4 years of social work experience in casework and counselling, preferably in mental health or geriatric field

- Demonstrate sound and updated working knowledge of healthcare and social services, their service delivery and relevant public policies

- Able to lead team and strong team player

- Independent, organized, proactive, with good working attitude and problem-solving skills

- Strong communication & interpersonal skills

- Able to align with organization's vision, mission and values

- Willing to work on some weekends for community events, when required
